1. Licensure, Telehealth Readiness & Compliance

What license do you hold (LCSW or LCPC), and in which state(s) are you actively licensed? Is your license active and unrestricted?

Are you legally authorized to provide telehealth therapy in the state(s) you listed? Any restrictions we should know about?

Do you carry your own malpractice insurance with at least $1M per occurrence / $3M aggregate? If not, will you obtain it before starting?

Describe your telehealth setup (private space, secure device, reliable internet). Any foreseeable barriers to consistent telehealth delivery?

2. Experience & Clinical Approach

Describe your experience conducting initial psychosocial assessments and creating treatment plans. What does a strong first session look like?

What therapeutic modalities do you use most often (e.g., CBT, ACT, supportive therapy, trauma-informed approaches)? What populations do you work best with?

Midlife MD focuses on midlife mental health (stress, anxiety, mood, life transitions). What is your experience working with these themes?

What types of clinical situations do you consider outside your scope for telehealth-only management, and how do you handle them?

3. Safety, Crisis & Mandatory Reporting

Describe your approach to suicide risk assessment in telehealth. What tools/frameworks do you use and what triggers escalation?

Give an example of when you referred a patient to a higher level of care (IOP/PHP/ER/in-person) and how you coordinated it.

Briefly describe your approach to mandatory reporting (abuse/neglect) and how you document/report safely and ethically.

4. Collaboration, Documentation & Patient Messaging

How do you coordinate care with medical providers (NPs/MDs) while maintaining therapy independence? Give an example.

Our expectation is session documentation completed within 24 hours. What does your documentation workflow look like?

Our expectation is responding to patient messages within 48 hours (excluding weekends/holidays). How do you manage timely messaging while maintaining boundaries?

5. Schedule & Fit

What is your ideal weekly schedule for a part-time telehealth therapy role (days/times/time zone)? How many sessions per week are you aiming for?
