Welcome to Midlife MD Clinic. By accessing or using our website and telehealth services, you agree to the following terms. Please read them carefully before booking or receiving care.
1. Payment, Insurance, and Services
Midlife MD offers services on a cash-pay basis and, for certain services and providers, through participating insurance plans. Availability of insurance coverage may vary by provider, service type, and your individual health plan.
Cash-pay services: For visits you elect to pay in cash (self-pay), payment is due prior to or at the time of service or through an active membership plan. For self-pay visits, we do not submit claims to insurance and no retroactive billing to insurance will occur for that same visit.
Insurance-billed services: For visits billed to insurance, you authorize us to submit claims to your health plan and to receive payment directly. You are responsible for any copayments, coinsurance, deductibles, and non-covered services as determined by your plan. Once a claim is submitted to insurance for a given visit, that visit is governed by your plan’s rules and our payer contracts.
No mixed billing for a single visit: A single encounter cannot be split between insurance and cash-pay in order to increase coverage or reimbursement. Each visit is billed either as cash-pay or as an insurance-billed visit, but not both.
Clinical medical services are currently provided only to residents of Illinois. All care is conducted via telehealth, with no in-person appointments unless otherwise stated.
2. Scheduling, Cancellations, and No-Show Policy
Appointments require a minimum of 24-hour notice for cancellation or rescheduling. Missed appointments or cancellations received with less than 24 hours’ notice may be subject to a $25 no-show or late cancellation fee.
This fee is not a medical charge and is not covered by insurance. It is the patient’s responsibility and must be paid prior to booking additional appointments.
Insurance-billed visits: Insurance plans do not cover missed appointments. We cannot bill insurance for a visit that did not occur, nor can we collect copayments, deductibles, or coinsurance for a missed appointment. Only the $25 no-show or late cancellation fee may apply.
Cash-pay visits: For cash-pay appointments, the $25 no-show or late cancellation fee applies when a patient does not attend the scheduled visit or provides insufficient notice.
Early departure: If a patient elects to end a cash-pay visit early, the fee is based on the appointment type and time reserved, not the number of minutes used. For insurance-billed visits, billing must reflect only the services actually provided.
3. Telehealth Limitations
Telehealth is designed for non-emergency care and wellness services. If you are experiencing a medical emergency, call 911 or go to your nearest emergency department. Our telehealth platform is not a substitute for emergency medical attention.
4. Scope of Care
Our clinicians provide care within their professional licensure and scope of practice under applicable law. We do not guarantee specific outcomes, and all medical recommendations are based on individual assessment and clinical judgment. Certain services may not be appropriate via telehealth and may require in-person evaluation or referral.
5. Privacy and Data Security
We take your privacy seriously. All data is managed in accordance with our Privacy Policy and stored within secure, HIPAA-aware systems. By using our services, you consent to the collection, use, and disclosure of your information as reasonably necessary for treatment, payment, and health care operations.
6. Disclaimers and Limitation of Liability
Services are provided “as-is” without warranties of any kind, express or implied. To the fullest extent permitted by law, Midlife MD Clinic and its clinicians are not liable for indirect, incidental, special, or consequential damages resulting from the use or inability to use our services or website. Nothing in these Terms limits liability where such limitation is prohibited by law.
7. Policy Updates
We may revise these Terms of Service periodically to reflect updates in regulations, participating insurance plans, or clinical operations. Continued use of our services after such updates constitutes acceptance of the revised terms. The most current version of these Terms will always be available on this page.
